St. Luke’s Health – Sugar Land Hospital Awards Grant to AccessHealth to Support Step-by-Step Program
RICHMOND, TX — AccessHealth is proud to announce that it has received a grant from St. Luke’s Health – Sugar Land Hospital to support the organization’s innovative Step-by-Step program, a comprehensive program focused on improving the prevention and management of diabetes and hypertension among medically underserved residents in Fort Bend County.
The Step-by-Step program integrates three evidence-informed interventions: food prescriptions, walking prescriptions, and chronic condition case management, into one patient-centered approach designed to empower individuals to take control of their health through preventive care practices and community support.
Through the program, AccessHealth will serve 50 participants over a 12-month period by providing monthly visits with a Community Health Worker (CHW), along with referrals to clinical and community-based resources. Participants will receive support that may include healthy food prescriptions, walking and exercise guidance, chronic disease education, and connections to additional services that address barriers to long-term health improvement.

The Step-by-Step Project was developed to address the growing impact of chronic disease within vulnerable populations across Fort Bend County. By combining healthcare services with practical lifestyle interventions and personalized guidance, the program seeks to improve patient confidence, increase self-management skills, and connect individuals with sustainable resources that support healthier living.
